The Drug Enforcement Administration (DEA), Office of Diversion Control, will accept requests from distributors that require a large volume of Order Forms (DEA Form 222) with the pin feed tracking left on the form. Order Forms are used for the distribution of a Schedule I or II controlled substance. These forms will be shipped in boxed form with pin-fed tracking along both sides for use in a dot matrix printer (the forms will not be separated in the normal seven forms per book format). Requests will be processed only on the first of each month with a minimum of 100 books and a maximum of 200 books per request.

Requests for this special ordering must include DEA number, name and address of registrant. The order forms will be shipped via 1st class mail, by the United States Postal Service. No provisions will be made for special handling.
